The scale of the problem

Traditional cryptographic hashes like MD5 and SHA-1 can only identify exact copies. Change a single pixel, compress a video, crop an image, or add a filter—and the hash changes completely. Harmful content evades detection simply by being slightly modified.
Perceptual hashing technologies like PDQ and PhotoDNA improved on this by detecting modified images—but they struggle with videos and fail when content is significantly transformed. They also produce many false positives, requiring extensive manual review.
The result? Known harmful content continues to circulate, victims are re-traumatized, and trust and safety teams are overwhelmed trying to manually review content that should have been automatically detected.
